Designate a Proctor

This form allows you to enter new proctor information or to change existing proctor information. If you are taking one or more courses, please enter ALL Course Designator and Number information in the appropriate fields.

Note: There's no need to submit this form if your proctor information is NOT new or changing.

Selecting "CMU Online - Mt. Pleasant" as your proctor means exams will be administered at one of our physical locations in Mt. Pleasant, MI: CMU North or the Park Library. Students using CMU Online as their proctor can elect to take their exams at either location throughout the term.
